Data Combination for ModMed Users
MIPSpro Data Combination Process for ModMed Users
📄 Overview
When ModMed users need to submit their MIPS report and have data from multiple systems, MIPSpro allows a user to combine all those data sources into one complete report for the full reporting year.
🆔 1. Patient ID Matching
All patient IDs must match across all of your data sources. If the same patient has different IDs in different systems, MIPSpro will treat them as separate patients and visits. This can cause duplicate instances and inaccurate performance results.
To help users combine data from multiple sources, Healthmonix created the Patient Matching feature for MIPSpro. This feature uses first name, last name, and date of birth to identify and match the same patient across all data sources.
Important:
- Patient Matching must be enabled before uploading any data.
- If data is already uploaded, it must be deleted, the feature turned on, and the data re-uploaded.
- There is an additional fee for this feature to be added to your account. If this is needed, you will need to contact the Sales team to confirm pricing and payment.
💾 2. Preparing Your Data
You’ll need to contact each of your previous EHRs or data sources to request an export of your MIPS reporting data.
Accepted file types:
- .csv, .xls, .xlsx, or .txt
- Advanced formats: QRDA I and CCD
Note: QRDA III files are not accepted because they lack the detailed coding needed for MIPS reporting. MIPSpro calculates your performance based on the codes included in your uploaded data, so it’s important that all required coding is included in your file.
🚀 3. Getting Started
Once your licenses are added to your account:
- Book your onboarding meeting with a support agent.
- Set up your ModMed MIPSpro account (Set up practice, register your provider(s), assign the license(s), complete the profile, and select Quality measures.
- Create separate accounts for each of your other data sources.
- Begin by uploading a sample (e.g., one month) of data from each additional source before uploading the full dataset. This helps identify and correct any issues early.
Helpful Resources:
✔️ 4. Validate Your Data
After uploading your sample data, complete a validation process to confirm that your data and performance results look accurate in MIPSpro.

🔗 5. Combine Your Data
Once the sample data is confirmed as accurate in each account:
- Upload the full dataset from each additional data source into your "Patient Matched" ModMed account in MIPSpro.
- This process will combine all your data into a single, complete report.
- Perform another validation check after combining to ensure your results are accurate.
📝 Quick Checklist
- Create a separate practice for each data source (ModMed, EHR1, EHR2, etc.)
- Have customer support agent enable Patient Matching if patient IDs don’t match
- Upload a sample of data from each data source
- Validate accuracy and performance for each source
- Upload all remaining data into your "Patient Matched" ModMed account to combine everything
- Complete one final validation to confirm the combined results
